WAIVER AND RELEASE FORM
I, the undersigned, hereby waive any and all claims against any and all persons associated with 10th Planet Pompano Beach & Coach Matthew Walsh.  I understand I am participating in martial arts, essential to which is body contact and that such participation includes, but is by no means limited to, the following: choke holds, arm locks, neck cranks, ankle locks, throws, slams, punches kicks, elbows, knees, and the like, any one of which could potentially cause serious injury or even death.  I recognize, moreover, that martial arts is an extremely dangerous sport and that minimizing the danger would defeat the purposes of the sport.  Accordingly, I assume full responsibility for any and all harm that may result to me as a consequence of my participation in all classes and events at 10th Planet Pompano Beach & by Matt Walsh, and accept the reality that it is highly likely that I will at some point suffer some injury as a result of the same.  Because I fully understand the risk and consequence of participating in the form of martial arts class offered by 10th Planet Pompano Beach, I hereby release Matthew Walsh, 10th Planet Jiu Jitsu Pompano Beach, and all of it’s agents, employees, associates, and independent contractors of and from any and all liability, claims, demands, actions, medical bills, and other liabilities whatsoever arising out of or relating to any negligent act or omission, and/or any loss, damage, or injury, including death, that may be sustained by the undersigned and/or the undersigned’s personal property as a result of such participation.  I the undersigned also state that I am in good physical condition and know of no reason why I can not participate in this physical activity.  Accordingly, I assume full responsibility for any and all harm that may result to me as a consequence of my participation in the activities of 10th Planet Jiu Jitsu Pompano Beach and Matt Walsh and do accept the reality that it is highly likely that I will at some point suffer some injury as a result of the same.  

In addition to the foregoing, I, the undersigned further agrees to abide by all rules established by 10th Planet Pompano Beach & Coach Matthew Walsh, including but not limited to, maintaining good and proper hygiene (freshly cleaned rash guards or other attire before each class, trimmed finger and toe nails, foregoing training with open wounds or sores, etc.). I understand that failure to abide by any such rules may be grounds for expulsion from 10th Planet Pompano Beach. Finally, I, the undersigned, further acknowledge that video cameras operate in 10th Planet Pompano Beach, and I grant permission to the rights of my image, likeness and sound of my voice as recorded on audio or video tape without payment or any other consideration. I understand that my image may be edited, copied, exhibited, published or distributed, including on the internet and in social media apps, and I waive the right to inspect or approve any such video or audio in which my likeness or voice appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording.

In signing the foregoing release, the undersigned hereby acknowledges and represents: that he/she has read the foregoing release, understands it and signs it voluntarily; that he/she is over 18 years of age and of sound mind.

What is 10th Planet Jiu Jitsu?

10th Planet Jiu Jitsu is a non-traditional system of Brazilian Jiu-Jitsu developed by Eddie Bravo. After earning a black belt under Jean-Jacques Machado, Eddie Bravo focused his jiu jitsu on training without the gi. The main premise behind the 10th Planet Jiu Jitsu system is that the art should keep evolving.

Matt Walsh - Lead Instructor

1st Degree 10th Planet black belt instructor, coach Matt Walsh, has been a devoted student of the system since 2008.

In 2013 he started teaching classes in 10th Planet gyms including premier moons like the ones in San Francisco and San Diego.

Coach Matt has years of experience in the world, national, and regional no gi competition scene having regularly taken the podium in all categories. In recent years his training focuses more on coaching and assisting students/ training partners in their competitive careers.

With an extensive background in allied health he devotes his efforts as a coach towards the wellbeing and longevity of his students.
